Transfer Forms
SAIC Common Stock Transfer Form
Use this form to transfer directly held vested shares of SAIC, Inc. common stock from your Computershare SAIC, Inc. common stock account to either an existing account or new account registered to an individual, joint or custodial account, trust, estate, charitable organization, etc.
- For transfers greater than 250 shares, a Medallion Signature Guarantee is required from any bank, investment house or brokerage firm.
- Transfers of 250 shares or less do not require a Medallion Signature Guarantee.
- Additional documentation may also be required to be submitted such as when creating a new trust account or in the event of death.
- If you are transferring shares to a charitable organization and the organization desires to sell the transferred shares, the charitable organization will need to submit the sale order itself, either through Computershare's Share Selling Program or by first transferring the shares from Computershare to a brokerage account.
SAIC Common Stock Transfer to Broker Form
Use this form to place brokerage account information on file at Computershare and/or transfer shares from your SAIC, Inc. common stock account at Computershare to a brokerage account.
- A Medallion Signature Guarantee is required from any bank, investment house or brokerage firm.
- Please note that only whole shares can be transferred from your SAIC Account at Computershare to a brokerage account.
Name Change Form
SAIC Common Stock Name Change Form
Use this form to change your name on your existing Computershare SAIC, Inc. common stock account due to a marriage, divorce, court order, etc. A Medallion Signature Guarantee is required from any bank, investment house or brokerage firm.
Stock Option Forms
Attestation Form - Stock Swap
The paper version of the Attestation Form is required if you are exercising your stock options using the stock swap (stock-for-stock) exercise method and you are submitting your option exercise transaction by telephone via a customer service representative or the interactive voice response system. If you submit your option exercise transaction online, you will be required to attest to your stock ownership for the stock swap exercise of the stock option within the online application and the paper version of the form is not required. The form represents your certification that you directly own the shares being swapped.
SAIC Stock Option Exercise Checklist
This checklist is recommended but not required. It should be sent with your payment for the option exercise and an Attestation Form. The Attestation Form is required if doing a stock swap and your option exercise transaction was submitted by telephone via a Customer Service Representative or the Interactive Voice Response system. The paper version of the Attestation Form is not required if you submit your option exercise transaction online.
If you are wiring the funds to Computershare, then fax this checklist along with your Attestation Form (if required) prior to wiring the funds.
