Complete the e-QIP application.
You have been requested to obtain a security clearance consistent with your performance on a classified contract. Your security clearance and future access to classified national security information will be based on the results of a personnel security investigation conducted by the Federal Government. Here are the steps you need to take to begin the process:
Complete a Standard Form 86 (SF86) via the Electronic Questionnaires for Investigations Processing (e-QIP) system.
Contact your local Facility Security Officer (FSO) found in your clearance email to:
Obtain your fingerprint cards (FD-258 format)
Verify your citizenship
Note: Please ensure you complete your fingerprint cards ASAP in order to not delay your security investigation. We will not be able to submit your SF86 to DISCO until your fingerprint cards have been received.

Before continuing to the e-QIP site (link at bottom of page), read or print this information and review the tips to successfully complete the application process.
1. Golden Questions
You must generate a user account by creating a username and password on your initial e-QIP login. You are required to answer a series of “Golden Questions” before you create your account:
You must type unknown if asked “In what city were you born?" All responses must be in lower case.
All subsequent logins will only require your username and password.
2. Employment Questions (Section 13) of the questionnaire
Enter this information into the following fields:
Current Employer: SAIC
Provide the Address of Employer: 151 Lafayette Drive, Oak Ridge, TN 37830
Employer’s Phone Number: 866-955-7242
Start Date: If you are completing this form prior to officially starting with SAIC, use the date that is listed on your offer letter as your start date
Physical Work Address: enter your actual work location
When prompted to enter your work location, which could be different from your employer address, please contact your supervisor or person who sent the offer letter to obtain the correct address.
Current Residence (for relocating employees):
If you are relocating for the position please provide a residence within commuting distance of your new work location.
When entering your current residence, please list a comment stating that you will relocate to your work location if your job offer is accepted, if applicable.
3. Signature Forms:
Employee will print the signature forms, sign them and forward to Security Services by one of the following methods:

If you do not have encryption capability please give to your local security office/Facility Security Officer and they can send them via encrypted email.
On Step #4 select “No” as you will not attach your signature forms.
Employee will click the “Transmit to Agency” link in e-QIP to forward the SF86 to Security Services, where it will be “Ready for Review” the following day.

- If you have problems viewing e-QIP, please check to ensure your browser settings are configured correctly. For assistance with browser requirements, please visit the Office of Personnel Management (OPM) e-QIP site.
You have five days to log in to the system and complete this form.
